From a long time ago...........
"We were on the 9.57 to Paddington to go and present to a potential client. I was suited (Next, since you didn't ask, bargain in the sale) and booted and wearing full metal jacket make-up wise, (i.e. wearing foundation and blusher, rather than just eye make-up which is my usual option if I'm in the office all day.) As the jacket of the suit I was wearing was shorter than my preferred style, I had opted for a rather skimpy thong underneath, in order to avoid the dreaded VPL.
We were travelling first class as it was only an extra £5, and the extra space makes it much easier to work, unlike those cramped seats with silly drop down flaps in standard class, or "steerage" as my colleague calls it. The other advantage is the nice man with the trolley who comes round with complimentary tea, coffee, pastries and bottled water. My daily intake of water is pretty high, but when in the office I limit myself to 1 cup of coffee a day. I had 2 cups of coffee and a bottle of water.
After a while I decided that I really should go for what the Americans refer to as a “comfort break”. Upon standing I realised that I should have made that decision earlier, as I was by now quite desperate. I made my way to the facilities, which thankfully were not engaged and were therefore available for immediate occupation. I entered, locked the door undid my trousers and assumed the position. Embarking with some relief on a much needed emptying of my bladder, I noticed that although my trousers were around my knees, they had not been accompanied by my underwear, which was still in its designated area.
I felt that I had 2 options, namely:
1) Attempt some very difficult and possibly limb threatening contortions to get the relevant parts under the hand drier.
2) Remove the garment and put it in the bin and spend the rest of the day "commando"
Dear Reader, I chose option 2, and can testify to the non-chafing properties of Next trousers."

I went commando due to a memory lapse in the days of mini skirts. When I realised I sidled out the door ( I was at work ) without telling anyone, sidled up to M&S, bought a pair of pants, and sidled back to the ladies toilet and corrected things. No one missed me, and I prayed that no one had noticed.
One of my daughters lived with her Nan for a while to be near her work. Her Nan was horrified by her scanty pants and called them "little bits of nonsense that won't cover possible".
I used to work with a really lovely girl (yes, we were both 'girls' then) who, as she stepped off the escalator at Tottenham Court Road, felt her knicker elastic 'go', She did no more than let them drop to the ground and deftly stepped over them!
ExaltedWombat why do you say "excessive" make up? I wasn't done up as Coco the Clown or Lily Savage on a hen do, just a light foundation, blusher and the usual eye stuff.
When I was doing that job I would only do my eyes for a day in the office (lazy mare, no foundation meant one less thing to have to take off at night!) and also go to work in fairly casual attire, but a meeting with a potential client was about looking professional and "polished"!
These days I go to work in jeans wearing no make up at all, but that's mainly because I can't see well enough to put my eyeliner on without a very strongly magnifying mirror, which of course just highlights every wrinkle, crevice and crack!

My elder sisters, now both in their 70's (just) still shudder at the memory of the day that my late mother stormed into school to check that my sisters were wearing knickers! 
I once went on a cycling holiday in france and didnt take enough underwear not expecting to be drenched in sweat every day (it was very hot.) All my clothes had to fit in saddlebags. After a few days I was forced to wash all my underwear (bras and pants) to hang and drip dry overnight in the hotel bathroom. So I had to go down to dinner in a (loosish!) blouse and skirt and sandals and nothing else! I was much younger then and felt I could get away with it.
In my perkier days
I once forgot to put a bra on when I went to work. It was a hot summer's day. I worked in a clearing room in a bank and spent a lot of time sat at a big computer station. That day I seemed to have a lot of male staff popping by for a 'word' (LOOK!) But the penny didn't drop until a lunchtime and a loo visit, when I caught sight of my reflection and suddenly realised. I was wearing a pretty lacey blouse with a demure neckline, but it was a little see-through. Needless to say, I quickly donned my jumper and sweated my way through the rest of the afternoon! I still blush now!
Phoenix, your original post made me laugh so hard it's as well I was at home. My only commando episode was recently at the pool on the way to work. I go in a swimsuit to save time changing, but then found it was easier to get into a dry swimsuit after, than wrestle with underwear, so I always packed one in my bag . . . except for the day I forgot. No time to go back home, so I'm there in front of a class of Year 6 hoping they won't notice that I'm unsupported up top. Fortunately I was wearing trousers but it was not the most comfortable day. Now I keep spare undies in the rucksack in case I forget again.
I was wearing new sloggies and became very sore in my groin so next day went knicker-less ,was wearing crops as it was very hot , went to local hospital for knee x-ray. Now last time I had knee x-rayed I didn't have to remove my crop trousers so was horrified when the young doctor told me to remove my trousers, my Portuguese being very basic I blabbed like a mad woman as to why I couldn't remove my trousers, I backed out of the x-ray cubicle and hobbled home to my husband who didn't see anything funny about my going nicker-less especially as I didn't get to the toilet in time while giggling like someone possessed.
Sheepishly went back to x-ray next day complete with knickers and red face.?
Am I the only one who admits to enjoy going commando style? To be fair, I only do it at home and usually in the summer months! And I'm no spring chicken either!!!
I was newly married and out shopping with DH in a discount clothing store. It was quite cold and I was wearing tights under my trousers. Always on the lookout for a bargain, I went into the changing room to try something on and had to remove the tights and trousers.
When I came out, my DH had bumped into his mother and my 14 year old BIL . We were chatting away, when MIL said, "What is that lying on the floor at your feet?" I looked down at the same time as young BIL. There on the floor were my knickers. Some how the had got caught up in my trouser leg, when I was trying to get the tights back on in the fitting room and had gradually worked their way down my trouser leg and landed at the feet of MIL. As my young BIL turned a pretty shade of puce, I quietly bent down without a word, as if this was the most normal thing in the world and stuffed them in my pocket. 
I have so far, not had a knicker episode, but a looooong time ago, when shopping with my mum, her knicker elastic went. Quick thinking on her part, she dived into a shop, where she knew the staff, asked for a stapler. In the loo she folded and stapled the top of her pants. Well done mum. ? and the repair lasted till she got home.
That happened to me in the Paris metro. We didn't wear tousers in those days and suddenly my knicker elastic went. The only thing to do was to step out of my knickers and stuff them in my bag. I was sooo embarrassed, I have remembered it to this day. Remember when we carried buttons incase the rubber button failed on our suspender belts and a button or 6p piece could mend it!
I had a very prim and proper aunt (goodness knows how she produced 3 offspring) whose waist elastic gave in her half-slip as we were standing at a zebra crossing in France on holiday. There was a market nearby and lots of people around. It was the end of the world for her - she went bright red as she stepped out of her germolene-coloured underwear and picked it up, just as everyone was crossing the road.
On the same holiday - we were camping in a rather primitive campsite in France - she was on the toilet when she was attacked by a rather vicious hornet, which then chased her right across the field. Again, bright red and sooo embarrassed. I was 10, but I remember it very clearly. She was suffering from constipation, I seem to remember, (which is why she had retired to the loo) although, given her primness, I don't know how I got to hear about this, but I asked her if she had really gone "ngngn" (straining noise, like delivering a baby?!?!). My mother told me to shut up; it was embarrassing enough for her sister-in-law as it was!
