The holders are designed to stop people from stealing a whole roll or two. Believe me, it was very necessary as people did help themselves to whole rolls. I was a cleaner in my student days and you would just not believe the amount of toilet paper that went missing!
I agree the cubicles are often too small and the holders badly placed and difficult to get paper out of.
Even worse in my opinion are the cubicles designed for people on crutches and in wheelchairs, where the toilet paper is hung up somewhere that is completely inaccessible from the actual toilet, which creates appalling difficulties for those who are less mobile.
