The technique is very well known, and has been for a long time. If you want to privatise, you have to run down the system first, and create a need, and there is no more essential need than good health and being pain free. Then you allow 'glimpses of a solution' - re-inforce that need, and defund some more. And bingo. It is all so tragically simple, sadly.
