I have always assumed that those who persist in referring to the Duchess of Cambridge as Kate Middelton are either opposed to Royalty or the reverse, such arrants snobs that they are determined to remind us all, including the lady in question and her husband that she was not born in a royal palace, but in a home that far more nearly corresponds to most people's.
I really don't think most people put that much thought into it. The title Duchess of Cambridge has just never really caught on in the public mind. Neither has Duchess of Cornwall or Duchess of Sussex. Times have just changed obviously.

I don’t care what the press call William’s wife, and think there’s too much drivel printed about the RF of whom in truth we actually know very little because of their culture of secrecy. I do agree with Alegria’s point about the way the press manipulate the public by the careful wording they employ in describing people and events.
I'm sorry, why is it considered a privilege to refer to someone by their name instead of a silly made up title that didn't exist for 200 years until the Queen resurrected it?

Catherine, be it spelled with a C or K became Duchess of Cambridge on the day of her marriage to Prince William. Maybe her nearest and dearest refer to her as K/Cate That is their privilege not that of the public.
I hate sloppy journalism where members of the royal family are referred to as if they are 'our family', they are not. I hated terms like 'Queen Mum', 'Lady Di', not because I am a royalist ( I am not) but because it is all part of the myth. I wish them no harm, but I do not 'love' or 'adore' people that I have not, and never will meet.
I doubt if the intention is disrespect. It is actually interesting to many people that an important member of the Royal Family and future queen is a commoner by birth and upbringing.
I’ve never believed you give up your own surname when you marry. You are still Ms Mary Smith, known as Mrs Mary Jones for practical purposes. I’ve always seen it like that but a lot of women seem to be proud to ‘lose’ their surname!
Doubt the Queen cares, at one time the press referred to Randy Andy. Diana was often Princess Di , even though not a princess and even after the divorce. The York’s were Andy and Fergie. The Queen Mother was The Queen Mum.
With all the rubbish in the press over the years why would the Queen care about the use of the name Kate
